Enrolling with Activities Unlimited
The enrolment form has undergone a complete redesign to improve the way in which we collect information about the level of support required to enable your child or young person to participate in activities. This includes your needs as a parent carer, and the impact of your caring role on your family. The new enrolment process has merged the assessment, and also the way in which we confirm the information you provide, namely:
The support needs of your disabled child or young person
The telephone validation questionnaire.
In peak times from March to July, the number of enrolments we receive each week is far greater than we can cope with, and the demand often results in a delay in families receiving a service from Activities Unlimited. This delay prompted our review of the whole process.
With your help we have redesigned the system and are able to launch a new model, which will do everything at the touch of a button. Of course the process is a lot more sophisticated than that but we are confident you will receive a service, which is both timely and responsive to your needs. If you are entitled to one of Suffolk County Council’s Short Break Local Offers you will know in just seven days.
What happens during the 7 days?
We will send an email to confirm that your disabled child or young person is now an Activities Unlimited member and provide you with a membership number to book activities that are promoted on www.activities-unlimited.co.uk. This is the Membership Offer to all parents and carers of disabled children and young people in Suffolk, and includes information, advice, guidance and support relating to services provided by the council. More importantly the Membership Offer allows you to purchase at a concessionary price specially promoted leisure activities.
The automated enrolment system is able to determine which disabled children and young people have moderate and higher levels needs and those who require additional support to be able to access all the services we provide under the Membership Offer. This support is known as a Short Break Offer or Short Break Offer Plus and is Suffolk County Council’s introductory Short Break Offer to disabled children and young people.
If you are offered one of the Short Break Offers you will be informed that you can request a higher level assessment if you so wish, which will be carried out by a social worker from our Disabled Children and Young Peoples team. All of your needs and those of your disabled child or young person’s physical, mental and social needs will be taken into account when services are being considered. In most cases we will implement the introductory Short Break Offer awarded to your child or young person but other services may be added to meet your specific circumstance and need.
I am not in receipt of a short break service so when will I know if I have one of the council’s short break offers?
To make certain that the automated enrolment system achieves a high standard, our family support team will look at all enrolments where it has been identified that the child or young person has a higher level need. This will allow the team to check the information you provide before asking the business support team to process the offer letter to you.
At regular intervals we will randomly sample all children and young people enrolments as part of our quality checks. Very occasionally you may tell us something about your child that we feel needs to be shared with another service who could help us in the care of your disabled child. We will ensure we seek your permission before we do this.
The checks we make
From 2018, we will be using a third-party provider (Experian) to carry out an identity validation check on our behalf. The nature of the AU service means we do not have the opportunity to meet with all of our customers face to face.
This is not a credit check, and we will not complete this check without your consent. In order to complete the check we need you to provide: your name, address, date of birth and, National Insurance Number (NI) to verify your identity. This is to safeguard the local authority and prevent potential fraudulent use of taxpayer's monies.
You will be asked for your consent to this check if you are awarded a financial offer from AU, this will be sent to you with the AU agreement form. If you do not wish to give consent for this check, please contact us to make an alternative arrangement.
